In accordance with the abovementioned Regulations, Article 11 requests can only be made by applicants (i.e. those that are proposing an activity and are unsure as to what waste authorisation, if any, is appropriate) or local authorities (where they have received an application for a waste authorisation and are unsure as to what waste authorisation, if any, is appropriate). I accept that the failure to provide any additional information (e.g. maps, supporting technical documents), as may be required, will result in the EPA making a determination based only on the information provided in this Article 11 request. Disposal/Recovery *		Is the proposed activity a waste disposal or waste recovery activity? More Information what constitutes Disposal and Recovery
Annual Residual Waste for disposal (Tonnes) *		1. State the quantity of residual waste that will be dispatched annually from your facility for disposal at another facility, following your recovery operation. 2. Please state the amount of residual waste, in tonnes for solid waste, and in litres and tonnes for liquid waste.

Supporting information, including engineer’s certification and/or material specifications may be sent to the Agency via email Article11@epa.ie or by post. The reference number for this request must be included with any submission.

Location map will be provided		A map (scale 1:10,000 or equivalent) showing the location of the proposed activity and the boundary outlined in red must be provided. The map should also show the extent of any existing or previous fill at or beside the proposed fill. This map should be forwarded to the following e-mail address Article11@epa.ie . Attachments must not be larger than 8Mb in size. The map may also be submitted by post. The reference number must be included with any submission. Please Indicate that you will provide a map.
